Welcome to the download area dedicated to the documentation required to participate in scuba diving, freediving, and mermaid courses.
In this section, you can download all the official PADI forms, which are essential to complete your course registration. Once downloaded, the documents must be:
- printed
- fully completed
- signed
After that, they must be sent via email using the link you will automatically receive upon registration.
In addition to the PADI forms, you are also required to download, complete, and submit the Underwater Academy forms, available by clicking on this link.
